Permission Agreement

I grant permission for my child to participate in programs in the Selleck Hall, the library, all classrooms, art room, science room, conference room, courtyards and gardens

I grant permission for my child to go to the playground, meadow, nature walks under the supervision of a SPDS teacher/staff member.

I acknowledge that I have read a copy of the SPDS parent handbook, the discipline policy and the abuse and neglect policy (located on the parent corner). I agree to abide by these policies and guidelines

I grant permission for my family’s name, address, email address and telephone number to be printed on the school directory.

I understand that photos and videos may be taken throughout the school year for internal communications and external marketing purposes. (no names will be published externally). By initially here, I acknowledge that I must submit a written request to Lisa Adorno if I would like to limit any photo/video permissions
